What are the advantages to home visits?

Medicare will cover the cost of physician home visits for patients who have difficulty accessing care in a clinic setting. Home visits have the advantage of providing improved access to care, improved physical functioning, and improved patient and family satisfaction. Evidence for in-home visits demonstrates reduced emergency department visits, hospitalizations, and reduced use of nursing homes. Remaining in one’s home provides clear cost savings over assisted living, residential care facilities, or other structured living environments. Plus, there are no transportation issues! With our extensive community connections, we can arrange for in-home blood draws, foot care, medication delivery, EKGs, X-rays, and ultrasounds, among other services.

What is a Geriatrician?

A geriatrician is a primary care physician who has completed a fellowship in geriatrics, or medical conditions unique to an aging population. Geriatricians have additional training in evaluation and management of conditions such as dementia, delirium, falls, frailty, urinary incontinence, polypharmacy (taking patients off of excessive medications,) palliative care, and end-of-life care. 

What is a Geriatric Care Manager?

Medical treatment is a sea of paperwork, voice mail, and red tape. The management of the complex, multiple medical issues of older adults can easily become overwhelming. A care manager is a person who can navigate these logistics on your behalf, allowing your family member to concentrate on enjoying life, and your medical provider to concentrate on managing actual medical problems.

What will it cost me?

Less than you think—and much, much less than an assisted-living facility. (Call for pricing.) Medicare Part B pays for physician services, and all medical visits are billed directly to Medicare. Care management services are typically not covered by medical insurance, although some long-term-care insurance policies may cover some portion of care management services.
