Services

Complete in-home histories and physical examinations done by Dr. Houts, board-certified in Geriatrics and Internal Medicine. Care includes a complete assessment, recommendations, and prescribing of medications, as well as ordering of any additional testing needed such as blood tests or x-rays. Medical services are billed directly to Medicare or another insurance provider.

Coordination of care including working with home health nurses, specialist physicians, therapists, and any other health care providers involved in your loved one’s care to ensure a cohesive treatment plan. Communication with family members, durable medical equipment companies, community pharmacies, and outpatient/insurance care coordinators to prevent gaps in care. Services are paid for out-of-pocket on an hourly basis – you will only pay for services that you actually use.

Complete medication management including ordering of medications, pick-up from the pharmacy, set-up in a medication reminder box, and weekly or bi-weekly check-ins for adherence, effectiveness, and side effects. This is a monthly out-of-pocket expense.

For a monthly out-of-pocket fee, you may choose to purchase 24/7 on-call services. This gives you around-the-clock access directly to our treatment team; no more answering services or “covering” providers who are unfamiliar with your loved one.
